Claudia Fine is a leader in innovative care for the elderly, chronically ill, and functionally challenged. In the early 90s she anticipated the need to take healthcare out of hospitals and long-term care institutions, and founded New York City's largest care management practice. The company was acquired and became SeniorBridge, a national home-care company.

 

At SeniorBridge, Claudia was responsible for the development of an enhanced approach to elder care, integrating traditional home care services with high level clinical consultation and professional management. Recognizing the value of SeniorBridge's unique model, Humana acquired the company in 2012 and created Humana At Home, an investment designed to reduce Humana members' high medical costs. The model was extremely successful, and improved the lives of Humana's Medicare Advantage members, allowing member with severe medical and functional challenges to remain safely in their homes.

Under Claudia Fine’s leadership, a training program was designed and implemented, which supported and educated thousands of nurses and social workers in the Humana Care Management Network. While Claudia Fine's work has reshaped the landscape of eldercare across the nation, she has remained passionately committed to working with individuals and families, as well as providing clinical supervision to professionals in the field.

Ms. Fine is a Certified Care Manager (CCM) and Case Manager (CMC) She has a Master's Degree in Public Health from Columbia University School of Public Health and a Master's Degree in Social Work from Hunter College School of Social Work. Claudia is also a member of the American Society of Aging and the National Association of Social Workers, as well as the former President of the National Association of Professional Geriatric Care Managers.
